You didn’t let go of your javelin or spear when you tried to throw it and you stab your own foot. Take 1d4 damage.

Dry fire

You miss nocking your arrow and dry fire your bow. The annoyance gives you disadvantage on your next shot.

Accidental cleave

If another opponent is within melee range of your target, your weapon springs off the first and does double damage to the second. No attack roll needed.

Accidental sharpening

Your swing is so perfect that your blade is sharpened on your opponent’s armor/hide providing you better accuracy on the next attack. Roll your next attack with advantage.
