On a melee attack, your weapon gets stuck in your opponent’s armor and you lose your grip on it. You must spend an action dislodging the weapon to recover it. When you do, you provoke an attack of opportunity.
If you tried a melee attack, you missed widely enough to provoke an attack of opportunity from your target.
If you were using a stringed weapon like a bow or crossbow, the string snapped. This weapon is useless until it can be repaired.
On a ranged attack, your projectile ricochets off a surface and hits another target. DM rolls to randomly determine whom — ally or foe. Same attack roll applies to the new target’s AC.
You create (or just now notice) a tiny sliver in between two of your fingers. You take 1 HP of damage and have disadvantage on all Concentration and Sleight of Hand checks for the next 1d4 rounds.
You hit your weapon on your knee. You’ll have disadvantage on any DEX checks for the next 1d4 rounds.
You miss your target entirely and land a critical hit on your nearest ally.
